What to expect as a new patient...
- Patient Medical History/Questionnaire Form (For first time, new patients)
Click on the link, open the form and type right into it. When finished, please review entire form making sure that all physician's names, numbers and contact information is included. Also, please be sure to list your current medications and their dosages, if applicable. After reviewing, please click the "print form" button on the bottom of the form. You can save a copy of the completed form to your computer for your records and bring the printed copy to your first appointment. If it is easier, print the entire form and complete manually. In general, for most procedures, following surgery, you will return for a follow-up visit to the office approximately 10 days to 2 weeks after your release from the hospital. At this time, your sutures or staples will be removed as long as the wound is healing appropriately.
